Review stars not longer appear in SERPs

Our healthcare directory which directs visitors to clinics lost it's review stars in SERPs.
Looking for any guidance on how to get this back.

Categories showing on SERP listings?
Hi I was wondering if anyone knows what these are called? See attached screenshot. Basically, it looks like Google is pulling the primary category and then sub categories from the site and adding them to the SERP listing. Are there any benefits to this besides possibly higher CTR? Sitelinks Not Appearing
We've have been ranking number 1 for all brand related terms but yet still sitelinks won't appear. We have submitted an xml sitemap We have a front facing sitemap in the footer We have a clear hierarchy of pages on the site We have a strong link profile We have a few hundred visits a month from organic search We are running Google Shopping ads Is there anything else that I should be doing?

Rich snippets not appearing in SERP
We implemented rich snippets for People listings about 2 months ago, and they haven't shown up in SERPs yet. I followed all of the advice listed here, and everything seems to be working and being crawled correctly. I'm not sure what else to try at this point. Why is this SERP displaying an incorrect URL for my homepage?
The full URL of a particular site's homepage is something like http://www.example.com/directory/.

The canonical and og URLs match.
The root domain 301 redirects to it using the absolute path. And yet the SERP (and the cached version of the page) lists it simply as http://www.example.com/. What gives? Could the problem be found at some deeper technical level (.htaccess or DirectoryIndex or something?) We fiddled with things a bit this week, and while our most recent changes appear to have been crawled (and cached), I am wondering whether I should give it some more time before I proceed as if the SERP won't ever reflect the correct URL. Knowledge Graph Quick Answer Box: Is there anything we can do to get our content to appear there?
Hi everyone, The quick answers box can be really helpful for searchers by pulling through content which answers their question or provides a clear description of an item or entity. Our client appeared in the quick answer box for a period of time with their description of a product, but have since been replaced by one of their competitors. Previously, the answer was provided by Wikipedia. Is there anything we can do to help get our client's content back in there? We've been looking at possible structured data we can use but haven't found anything. Also suggesting our client ensures they have a paragraph within their copy which is a clear, concise description of the product that Google can pull. Can anyone give any suggestions? Thanks Laura

Rich snippet star ratings appearing and disappearing
For nearly a year, our physician profile pages have been successfully displaying rich snippets in Google search results. Nothing has changed on those pages, but the stars recently vanished from Google. Example of a physician profile: http://www.realpatientratings.com/Lori-H-Saltz/ We added some new content pages in a different schema category, and those new pages actually do have rich snippets showing in Google search results. Example of a new product page: http://www.realpatientratings.com/breast-augmentation-reviews.html Furthermore, the Google Webmaster Tools Structured Data report has me convinced that the Physician profiles aren't coded correctly because it doesn't appear to recognize the Review or Aggregate Rating counts. But the official Structured Data Testing Tool says all is correct. My instinct is that the code isn't quite right on the physician profile... but I'm not a developer. New links appeared, how do I test to see if they are good or bad?
I've just noticed 5 links appear via Majestic. Opensiteexplorer hasnt picked these up yet. I want to check if I should get these removed or leave them be. How can I check to see if the link is good or bad?
