Manipulative Site Spam - Ideas to Fix?

Hey,
We have been asked to fix up a website which has been poorly optimised with manipulative techniques.
The company is a local service based business, and the old SEO company created thousands of 'pages' with a list of locations & services.
**Examples - **
domain.com.au/suburb1/service1
domain.com.au/suburb1/service2
etc
domain.com.au/suburb2/service1
domain.com.au/suburb2/service2
These pages dont actually exists and appear to 301 redirect back to domain.com.au/service1 (this is a real page with real content). The problem is that all of the above mentioned non-existent pages are actually all indexed.
Problem: How do I remove a page from the index that doesnt actually exist but has been 301d?

Make a way for Google to crawl the 301 pages and they should drop out naturally.

There's the manual request in Web Master Tools.
Though that they're indexed and not the 301 of the page seems odd. Make sure there is a crawlable link to each page somewhere in your site, perhaps even make sure they are still in your sitemap, and not blocked by robots.txt. That should allow google to re-crawl the pages and realise they have been 301.
Also check how the 301 is implimented. Make sure theres not some kind of masking that is redirecting users and not google. Also make sure it is a 301 and not a 302.

Staging site and "live" site have both been indexed by Google
While creating a site we forgot to password protect the staging site while it was being built. Now that the site has been moved to the new domain, it has come to my attention that both the staging site (site.staging.com) and the "live" site (site.com) are both being indexed. What is the best way to solve this problem? I was thinking about adding a 301 redirect from the staging site to the live site via HTACCESS. Any recommendations?

How Often is Site Crawled
Good morning- I saw some errors in my first crawl and immediately removed the pages from my website. I then re-created my XML sitemap and uploaded to Google. The question I have is will the site be crawled to recognize the changes in the next day or so? The pages were just placed on the site as test pages and never removed. The initial crawl that notified me it was done found the errors and were removed. Thanks for your help. Peter

Site Wide Links
I have a link on pr 3 home page website placed in the side bar. It is on a WordPress website that spans a couple hundred pages and the side bar is on every page. The majority of the pages are not ranked or have any pr. Can this affect me negatively?

What to do next with my site gamblingsites.co
So I have this site gamblingsites.co, which I launched about a year ago (I think.) This used to be internetgamblingsites.net (a domain I bought, but never managed to get in the index, and it appeared to violate the T/Cs after asking in GWMT) and before that the site used to be casinowarehouse.eu. After moving to gamblingsites.co, the pages were indexed almost instantly. I kept a 301 in place until today as I had some links pointing to internetgamblingsites.net. Now, until a few weeks ago, everything was fine. The site was ranking top 10 for gambling sites (8-10) and I had some traffic everyday. This site wasn't my top priority, so besides adding new unique content, I didn't do much with it. In each case no shady link building or what-so-ever. On February first of this year, however, it lost all of its rankings, and I have no idea why. Much worse site appear in the top 50, where a sub page of my site appears somewhere on the 9th SERP for keyword 'gambling sites.' Last week I started contacting some people and asked them to update my links. I also used my own sites (all on unique hosting accounts) to build some branded links, i.e. 'GamblingSites.co' and similar terms to down tune the exact match. I also decreased the instances of the exact match on the homepage, to avoid over optimization. Finally, I removed the 301 from internetgamblingsites.net, since the better links have been changed (or are about to get changed soon.) Now, couple of days later... no changes, but it's probably to early to judge. My question to you: "What would you do next, to try to save the site and at least get some traffic to it?" Site maps, Is there any benefit?
I have a relatively simple and small site (60 pages). All of it is crawlable and there is nothing I want non follow. So, is there any real benefit to a sitemap since Google can get to all the site anyway? Do they give the site more credence or something because it's there? I guess as an aside, are there any favorite sites that will generate a site map? Thanks!
