Why are rich snippets not appearing?

I'm helping out on a site (www.treadmillreviewguru.com) which formerly had rich snippets appearing in Google. They use hCard markup. All of the markup is visible on the page. The search preview in Google's rich snippets testing tool show a picture, a rating and all that.
However, nothing is appearing in search results. Here is an example page: http://www.treadmillreviewguru.com/nordictrack-treadmill-reviews/nordictrack-commercial-1750-treadmill-reviews-ratings/
This page ranks in the top ten (for me, global results) when I search [nordictrack commercial 1750 treadmill review].
Does anyone know why Google would display the rich snippets a few weeks ago and then not display any at all?

Google seems to be selective in displaying the rich snippets in results. I have a site that uses ratings snippets and only a tiny percent of the SERPs display with the rating stars. I can't figure out why some display with snippets and some without (# of ratings, rating score to not appear to factor), there must be something in the algorithm that decides this but it is not clear to me.

Our root domain is no longer appearing in search results
Hi all The root domain for our site, roadtrippers.com, has been disappearing from Google's search results. Subfolders and subdomains still appear, but our root domain isn't found at all. I believe I've verified this by searching "-inurl:trips -inurl:byways -inurl:support -inurl:blog -inurl:places -inurl:guides -inurl:destinations site:https://roadtrippers.com/" in Google and our root domain is nowhere to be found. This may or may not be related to another issue we've had, where the root domain is appearing with a seemingly rotating set of parameters. Sometimes it'll be ?mod=, sometimes it'll be ?tag=translation. Originally they appeared to simply displace our ranking root domain, but now they and our root domain are completely disappearing. Our dev team believes they fixed the problem with recent 301 tags to any unapproved parameter being added to the root domain, but this hasn't fixed the original problem. Any insight into this is greatly appreciated! Brandon

Same page but appearing in Google with different titles
I have a page ranking on position 1 for a key phrase. The key phrase is the title of the page as well. I'll use a mock key phrase to aid my question - "Teeth and Gums" So the page is ranking number 1 for "Teeth and Gums" and "Teeth and Gums" is the meta title. However, I went ahead and did a new search adding an additional keyword to the original search. When I did a new search adding an additional keyword to the original search, Google has done something weird.. Let's say the search is "Dentistry - Teeth and Gums", Google has ranked my page again as number 1 but changed the title. The title in the search result is now "Dentistry - Teeth and Gums" How and why? It's kinda like Google PPC's keyword insertion but the title hasn't got anything weird like {KeyWord: Dentistry}. It's just "Teeth and Gums" Has this happened to you guys? Any ideas?

Does anybody know of solid statistical examples of websites improving CTR via rich snippets?
There has been a fair deal of broad speculation that "rich snippets improve CTR". The recent rich snippet infographic that SEO Moz came out with mentioned this. However the only solid examples I could find was from an example on Search Engine Land. http://searchengineland.com/how-to-get-a-30-increase-in-ctr-with-structured-markup-105830 Does anybody know of any other success examples with solid data of improved search share CTR rates when rich snippets were implemented?

Is URL appearance defined by crawling or by XML sitemap
I am having a problem developing a sitemap because I have long URLs that are made by zend. They go like this: http://myagingfolks.com/professionals/20661/social-workers/pennsylvania-civi-stanger Because these URL's are long and are fed by Zend when I try to call them all up, to put on the sitemap, the system runs out of memory and crashes. Do you know what part of a search result, in google, say, comes from the URL? Would it be fine for me to submit to google only www.myagingfolks.com/professionals/20661. Does the crawler find that the URL is indeed http://myagingfolks.com/professionals/20661/social-workers/pennsylvania-civi-stanger or does it go with just what the sitemap tells it?

Will google punish us for using formulaic keyword-rich content on different pages on our site?
We have 100 to 150 words of SEO text per page on www.storitz.com. Our challenge is that we are a storage property aggregator with hundreds of metros. We have to distinguish each city with relevant and umique text. If we use a modular approach where we mix and match pre-written (by us) content, demographic and location oriented text in an attempt to create relevant and unique text for multiple (hundreds) of pages on our site, will we be devalued by Google?
