Websites being hacked & duplicated, what should we do?

Hi, please help!
Our website was hacked and being totally duplicated. They even injected codes to intercept our orders. Although the codes issue had been solved, still there're two mirror sites out there.
When search for some of our key words, they even have good ranks. What exactly can we do to let Google ban those two sites.

Duplicate Title Tags How harmful is it?
I recently updated a site to use a new WordPress theme. This theme in conjunction with Yoast SEO plugin is causing duplicate Title tags. The tags have the same title in them. I discovered this when adding new keywords and pages to the page optimizer in moz. I have since turned off moz to stop the duplicate title tag issue however I am wondering if tuning off yoast is worse then the duplicate title tags. Any clarification on the duplicate page title issue and its consequences would be greatly appreciated.

Non Moving #1 Website
Hi Guys, Bit of an odd one we have recently undertaken our SEO in house (6-7 Months) and since doing this have had some great results, we have identified our competitors utilising Ahrefs, Moz and other tools that we have selected. However we have come up against something that we really cannot work out. One of our competitors ranks for everything related to there term on their website and industry at position #1 They are not actively doing SEO, Poor backlink structure from the research that we have carried out No fresh content being generate I was wondering if someone would perhaps mind spending 5 mins having a quick look to see what you think as i can't think what we might have missed. How different should content be so that it is not considered duplicate?
I am making a 2nd website for the same company. The name of the company, our services, keywords and contact info will show up several times within the text of both websites. The overall text and paragraphs will be different but some info may be repeated on both sites. Should I continue this? What precautions should I take?

How to protect against duplicate content?
I just discovered that my company's 'dev website' (which mirrors our actual website, but which is where we add content before we put new content to our actual website) is being indexed by Google. My first thought is that I should add a rel=canonical tag to the actual website, so that Google knows that this duplicate content from the dev site is to be ignored. Is that the right move? Are there other things I should do? Thanks!

Duplicate Page Titles
I had an issue where I was getting duplicate page titles for my index file. The following URLs were being viewed as duplicates: www.calusacrossinganimalhospital.com www.calusacrossinganimalhospital.com/index.html www.calusacrossinganimalhospital.com/ I tried many solutions, and came across the rel="canonical". So i placed the the following in my index.html: I did a crawl, and it seemed to correct the duplicate content.
