Keyword Lists - Omitting Keywords

I've had trouble adding keywords to Keyword Lists - specifically when I add a large list of keywords (400+), and only 50 or 75 keywords are analyzed.
Is there a reason this happens?

We have a page on our website with a higher than average number of pageviews, 85% of which came from Google organic search. When I research this page by entering the URL into the "exact page" keyword research tool, Moz says it has no ranking keywords. Why it's an issue:Although using targeted keywords in H1 tags on your page does not directly correlate to high rankings, it does appear to provide some slight value. It's also considered a best practice for accessibility and helps potential visitors determine your page's content, so we recommend it. Over-using keywords, however, can be perceived as keyword stuffing (a form of search engine spam) and can negatively impact rankings, so use keywords in H1 tags two or fewer times. To adhere to best practices in Google News and Bing News, headlines should contain the relevant keyword target and be treated with the same importance as title tags

Does Moz's keyword tool pull data from your IP address?
Does anyone know how Moz's keyword tool pulls their keyword ranks? Do they take it based off of the IP (history and cookies) that is being used? I am trying to find a way to collect keyword data that is neutral and not based off of my previous searches, etc. TIA

National or local tracking of keywords
I manage an account with +20 locations and have used the default National tracking on keywords, but realized that we might see higher rankings if I choose local tracking. If any of you have any experience in this, please tell the world!

Keyword Rankings vs. Rank Tracker research tool
I have a campaign tracking several keywords. On my Keyword Rankings report I'm not in the top 50 for anything. When I use the Rank Tracker tool, my rankings are quite high (I have 2 in the top 5). When I use the Rank Tracker tool I've noticed that I'm able to point to specific URLs on my website (not the main page). When looking on my Keyword Rankings report, the URL is usually empty. So what am I missing? Have I configured something incorrectly? Can I update the URLs on the Keyword Rankings report so the rankings will be reflective of the Rank Tracker tool? OSQdLmb nMnOQim

On Page Grader - We have keyword in Meta Description But does not show up
The on-page grader is not counting/picking up the keyword in the meta description. We have double checked the meta description and re-uploaded the page, and checked the source code on-line.
