Keyword question

In my keywords should they include the city name or not?
Example..... plastic surgery or plastic surgery Orlando
I know the search engine knows what city i am in, if i search for plastic surgery and i live in Orlando, does it pull up world wide plastic surgery links first or local plastic surgery clinics?

Yes, you should definitely include the city name. A search for just "plastic surgery" is too broad and much less likely to pull up your particular business.
When I type in "plastic surgery" in my search, not a single web page contains a local result for plastic surgery in the City I live in (although several of the paid ads show up locally).
Local Places pages do show up for local businesses, but I'm assuming you're talking about pages on your website and not Google Places.

Phoenix Car Wraps One thought is that the keywords are so similar that it would best benefit us to target one page to the grouping of like keywords and long-tail keywords, and not making separate pages as to create competition for the same keyword within the site itself. Also that Google is "smart" enough to see that car wraps and vehicle wraps are like keywords, and that car wraps when linked to a site from Phoenix, will also pull as Car Wraps Phoenix. The other thought is we target one page for each separate keyword although the keywords are so similar. What are your thoughts? Hi Should I be looking to keyword every blog post I write? Obviously the point in a blog for most of us is to draw in traffic so should I keyword research every blog post and use that as keyword? What if there is no keyword stats for the blog im writing? I guess I just use a keyword I make up anyway and hope people type it? Thanks Chris

Meta Keywords Dilution?
In general, does having a large number of meta keywords listed in a page's meta keywords line dilute effort? On other words, should I focus on optimizing for 1 or 2 keywords per page to keep my efforts focused and increase the probability of ranking better for those 1 or 2.....or should I put down all the keywords I would "like" to rank for? Location-based Keyword Targeting
We are located in Denver, Colorado. I want the majority of the site to be focused on Denver keyword terms but I also think we should branch out to other cities around Denver, as well as around the keyword "Colorado." What's an appropriate way to do this? I've seen site that have a page for every city in the area but it looks terrible and doesn't really fit into the site. How do I cross link to these new city pages (I don't want them in the main navigation)?
