Can hosting blog posts with keyword anchor text on outbound links cause a penalty?

My site received a Google penalty for having inbound links from blog posts with over-optimized ("spammy") anchor text. I spent months getting these links removed.
Yesterday - I received a link deletion request from a site that my site had linked out to (three links via keyword anchor text relevant to their company) in a blog post.
The "unnatural link" penalty still hasn't been removed from my site.
My question is: Does the penalty work both ways? For having inbound "unnatural" links ... AND for having outbound "unnatural" links?

If you received an inbound unnatural links penalty then the links going OUT from your blog will not matter. It's possible for you to also get an outbound unnatural link penalty as well but you would receive notice of that just like you did with the inbound penalty.

Another client copies everything to blogspot. Is that what keeps her site from ranking? Or what? Appears to be a penalty somewhere but can't find it.
This client has a brand new site: http://www.susannoyesandersonpoems.com Her previous site was really bad for SEO, yet at one time she actually ranked on the first page for "LDS poems." She came to me because she lost rank. I checked things out and found some shoddy SEO work by a very popular Wordpress webhoste that I will leave unnamed. If you do a backlink analysis you can see the articles and backlinks they created. But there are so few, so I'm not sure if that was it, or it just was because of the fact that her site was so poorly optimized and Google made a change, and down she fell. Here's the only page she had on the LDS poems topic in her old site: https://web.archive.org/web/20130820161529/http://susannoyesandersonpoems.com/category/lds-poetry/ Even the links in the nav were bad as they were all images. And that ranked in position 2 I think she said. Even with her new site, she continues to decline. In fact she is nowhere to be found for main keywords making me think there is a penalty. To try and build rank for categories, I'm allowing google to index the category landing pages and had her write category descriptions that included keywords. We are also listing the categories on the left and linking to those category pages. Maybe those pages are watered down by the poem excerpts?? While SEOMoz currently can tell us the number of linking c-blocks, can SEOMoz tell us what the specific c-blocks are?
I know it is important to have a diverse set of c-blocks, but I don't know how it is possible to have a diverse set if I can't find out what the c-blocks are in the first place. Also, is there a standard for domain linking c-blocks? For instance, I'm not sure if a certain amount is considered "average" or "above-average."

We registered with Yahoo Directory. Why won't this show up as a a linking root domain in our link analysis??
Recently checked our link analysis report for 2 of our campaigns who are registered in the dir.yahoo.com (yahoo directory). For some reason, we don't see this being a domain that shows up as linking to our website - why is this?

Keyword cannibalisation
We created a product blog page that was highly optimized for SEO based on a recommendation from a colleague. These are now our best performing pages - however they do not convert as highly as the bona-fide product pages. After further investigation we're concerend that we shouldn't have split our content accross two pages - keyword cannibalisation. Is this correct and should we 301 our product blog pages to the other high converting pages?

Keyword cooccurence
Is there any truth to keyword cooccurence being a ranking factor in google or any other search engines. Meaning that they not only look at the words you are targeting but the other words on the page to see if those words have a close relationship to the main theme ( the words being supposedly targeted) I heard that keyword coocurrence does not scale well over the billions of pages that are on the internet however, some waterdowned approached of keyword cooccurence is possible. what do you guys think?

Keyword stuffing
I'm working on this page at the moment - http://www.gear-zone.co.uk/berghaus-bid6.html. The keyword we're going for is, unsurprisingly, 'Berghaus'. The SEOmoz on page report has flagged up a possible issue with keyword stuffing as the term is mention around 25 times. Is it worth altering this, which could be quite tricky as a lot of the page is dynamically generated? Thanks!
