Inurl: search shows results without keyword in URL

While doing some research on the indexation status of a client I ran into something unexpected. I have my hypothesis on what might be happing, but would like a second opinion on this.
The query 'site:example.org inurl:index.php' returns about 18.000 results. However, when I hover my mouse of these results, no index.php shows up in the URL. So, Google seems to think these (then duplicate content) URLs still exist, but a 301 has changed the actual goal URL? A similar things happens for inurl:page. In fact, all the 'index.php' and 'page' parameters were removed over a year back, so there in fact shouldn't be any of those left in the index by now. The dates next to the search results are 2005, 2008, etc. (i.e. far before 2013). These dates accurately reflect the times these forums topic were created.
Long story short: are these ~30.000 'phantom URLs' in the index out of total of ~100.000 indexed pages hurting the search rankings in some way? What do you suggest to get them out? Submitting a 100% coverage sitemap (just a few days back) doesn't seem to have any effect on these phantom results (yet).

We encountered something similar when we migrated a site. We properly redirected all the old url's to the new one, however, in the weeks after the migration, we saw a huge increase of 404 in the webmastertools.
When we took a closer look to these url's, we noticed that these where using an url structure we had abandoned several years ago. On the "old" site, these were redirected, but we didn't implement these old redirections after migration as we assumed that these very old url's wouldn't be in the index anymore. We proved wrong. We could delete them manually from the index using webmaster tools, because they used folders we are not using any longer, this is not probably not possible in your case.
While it is a bit annoying, I don't think that having these "phantom" url's in the index is doing you any harm in terms of SEO. They will probably never pop-up for normal search queries, only when you do in-depth queries, limiting the results to only your site.

A few days plus ~100,000 decade's old pages in Google is usuallynot enough time to see a change. You can spot check the 301s and run a fetch / render from GWT to see if the changes should be working though. Other than that, you'll probably have to wait a bit longer.

301 Redirects, Sitemaps and Indexing - How to hide redirected urls from search engines?
We have several pages in our site like this one, http://www.spectralink.com/solutions, which redirect to deeper page, http://www.spectralink.com/solutions/work-smarter-not-harder. Both urls are listed in the sitemap and both pages are being indexed. Should we remove those redirecting pages from the site map? Should we prevent the redirecting url from being indexed? If so, what's the best way to do that?

Why HTML entities gets crawled as content keywords in Google search console?
My Google search console shows HTML parameters such as div, class, img, src, gif, align as content keywords, but why google crawls HTML parameters as keywords? because of this, I would be losing traffic for my on-page content keywords. Please let me know how to solve this. Thanks, Jenifer

Website no longer visible Search Results
Overnight my website no longer appears in search engines for the two keywords I use. The website has been nicely climbing up (very steady progress to 42 and 73) the overnight it has vanished off the Radar. I have checked my webmaster account, no messages etc. Please can anyone shed any light on why this has happened? Do you only allow one URL (link) per Keyword?
I'm basically working with an outsourced SEO company. They fix 20 on-page links every month. To broaden my point, I'd like to know how to check up on their progress by using MOZ. Do you have any advice? Also, would we be able to use more than one link per keyword? I don't see this option but maybe you can help me with any suggestions. Thanks! Nzo Tiano

Structure of urls
**Hallo from Athens, Greece. We have to implement the following project and i need your help: ** We will build a company guide for the whole country and company local guides for each city for the same client. **Information of the country guide is the sum of information of local guides, so when a user is at the country guide he sees information from companies from all cities and when the user is at city guide he sees info only for the city. ** The problem is the structure of the url we should have. Should the page of presentation of each company should have structure as domain.gr/id/company? or city.domain.gr/id/company and the one to be canonical to the other? is this good for seo? How to alter the search result to this?
When searching for "kredittkort" on Norwegian Google I get a search results that looks like this. I want to replicate this, but I'm not sure what information they've provided and how they've done it. It's seems like their both listing products AND have sitelinks connected to a subsite. How is this possible? The sitelinks aren't even subpages of the ranked site. How have they managed this? Also, is the product previews they have?

Special characters in URL

For instance: www.mydomain.com/mykeyword1-mykeyword2%2C-1%2Cpage1.html I am about to make 301 redirect rules for all these urls to clean ones. IE: www.mydomain.com/mykeyword1-mykeyword2%2C-1%2Cpage1
