Seeing URL Slugs as search result titles

I've been seeing some search results for my site that look like the first result here, where the URL slug is used as SERP title: https://drive.google.com/a/fitsmallbusiness.com/file/d/0B37y4RslpuY-a0hQYjlJQ0NxeFJicDF6RVlURFVSNFN0aGhB/view?usp=sharing
The article title (and Yoast snippet title) are both "28 Press Release Examples From The Pros", but for some reason I'm seeing "press-release-examples" in the search results. I've seen this for multiple articles, and I see it now and then with different articles. I'm aware that Google often changes the titles in search results, but it seems very weird to me that they would opt for just the URL slug here.
Thoughts? Has anyone else seen this issue? Any idea what might be causing this? All help much appreciated.

Not a problem with link structure. Doesn't appear to be a problem with code, either, from a glance just now.
The good news? Your title tags work fine for me in Google search results and I see you at #1 ranking after the image pack for "press release examples".

IMO this is probably a personalized search result problem that you're seeing, and I would guess it's limited to you unless you've confirmed it with other people outside of your company computers.
Never hurts to resubmit the URL in Search Console, either.

Given that our page is 15/15 on those best practices, I don't think link structure is the issue here – unless there's something I'm missing. The URL is fitsmallbusiness.com/press-release-examples/

URL Format
Often we have web platforms that have a default URL structure that looks something like this www.widgetcompany.co.uk/widget-gallery/coloured-widgets/red-widgets This format is quite well structured but would it just be more effective to be www.widgetcompany.co.uk/red-widgets? I realise that it may depend on a lot of factors but generally is it better to have the shorter URL if targeting the key phrase "red widgets" One thing, it certainly looks a bit keyword stuffy with all those "widgets"

What is the best practice to seperate different locations and languages in an URL? At the moment the URL is www.abc.com/ch/de. Is there a better way to structure the URL from an SEO perspective?
I am looking for a solution for using a new URL structure without using www.abc.com**/ch/de** in the URL to deliver the right languages in specific countries where more than one language are spoken commonly. I am looking forward to your ideas!

Google Search Results and Trailing Slash
I noticed that when I search a page; Google is returning my search results without a trailing slashing. I have my site setup to adding a trailing slash and redirect to one if it is not. So http://www.oxygenconcentratorstore.com/inogen-one-g3 redirects to site:http://www.oxygenconcentratorstore.com/inogen-one-g3/ I just did a search for site:http://www.oxygenconcentratorstore.com/inogen-one-g3/ and the only result is the non-trailing version. See attached. All internal links and sitemap include a trailing slash. Should I be concerned about this?? I am concerned that it could be hurting my search results. Why "title missing or empty" when title tag exists?
Greetings! On Dec 1, 2011 in a SEOMoz campaign, two crawl metrics shot up from zero (Nov 17, Nov 24). "Title missing or empty" was 9,676. "Duplicate page content" was 9,678. Whoa! Content at site has not changed. I checked a sample of web pages and each seems to have a proper TITLE tag. Page content differs as well -- albeit we list electronic part numbers of hard-to-find parts, which look similar. I found a similar post http://www.seomoz.org/q/why-crawl-error-title-missing-or-empty-when-there-is-already-title-and-meta-desciption-in-place . In answer, Sha ran Screaming Frog crawler. I ran Frog crawler on a few hundred pages. Titles were found and hash codes were unique. Hmmm.
