Local pack ranking anomaly -- help?

At a bit of a loss on this one... If anyone has any ideas about what's going on or how to tackle this, I'm all ears.
One of my clients, an orthodontist, is appearing in the top three organic positions and in the local pack for almost all keywords we're targeting. However, for the keyword "orthodontist" without any location modifiers attached to it, we're appearing in the top three organic results but our Google listing is not appearing in the local pack.
The three listings appearing in the local pack are his next-door competitor, one practice that closed almost a year ago, and a practice in two towns over. He and his competitor are the only two orthodontists in this town, so they should theoretically be the two main listings that are being pulled in.
The listing for the closed practice is marked as closed on Google and has been reported to Google several times in the last few months. The listing has no website or reviews on it, although it does have an address and a phone number.
We have spent months doing aggressive, in-depth NAP/local listing cleanups. We have 24 Google reviews with an average rating of 4.6 stars, and we're organically gathering reviews every week. We went through a site redesign at the beginning of this year, so we now have a mobile responsive website.
We are appearing in the local pack for almost every other keyword that we have high organic rankings for, so we know it isn't necessarily an issue with our Google My Business listing.
Does anyone have any ideas of what's going on, or what we can do to get our listing to appear in the local pack for this keyword? The keyword "orthodontist" is the single most important keyword to this client and our strategy, so we're open to any and all suggestions or thoughts.

As Joy mentions, the best bet for getting targeted advice here would be to share the identity of this client and the precise search you're trying to rank for. Otherwise, we can only speculate. The possibilities could range from the Possum filter to whether you've got the right categories, to a lot of other things. If you're unable to mention the client, that's perfectly okay, but it does leave the community here guessing rather than investigating.

Can you tell me what city or zip you're searching "orthodontist" from so I can replicate it? Also, what's the website for your client?
It sounds like he's being filtered due to Possum but I would prefer to verify that by looking.

Different results & page layout in Google local for plurals?
I realize that the general results websites can rank differently for singular vs plural keywords, but today when checking local rankings for a client, I noticed that not only does the client rank differently for "church in San Diego" vs "churches in San Diego" but the layout and info of the local results pages are different. "Church in San Diego" (screenshot) shows the phone number and has links for Website and Directions "Churches in San Diego" (screenshot) doesn't show any of that instead has an image. If you click on the image, it brings up a card almost like a popup with info, reviews, and links for the organization. Anyone ever noticed that before? Anyone know why the difference? And if there are different optimization strategies?

The chicken community needs your helpful ideas
Hey Mozzers! We are currently working on reputation management for a client that sells chicken coops. They've gotten a few nasty reviews from one of the most popular community forums. Given the high DA it will be hard work (understatement of the year) to outrank that forum thread enough to knock it off the first page of SERPs. I've been brainstorming some unique ideas for the chicken community that could bring a more positive rapport to the product. We've considered an interactive game, polls, content for chicken communities and publications. Mozzers always have such fantastic insight, I thought I'd open up the brainstorm and see if anyone has a great, unique idea to drive traffic and increase brand engagement and awareness! Google Local Results - Incorrect Listing Url's
I have decided to re launch a small side project that I had to abandon a few years ago. I want to specifically target Google Local Results. I do need to overcome a proximity to centroid issue in a fairly competative niche. Unfortunately a number of my listings i.e. Facebook and Yelp have less than optimal URL's. For example: https://www.facebook.com/businessname.targetlocation.keyword/

Now I know that its best to keep the info consistent across the board, so I wonder if I would be better off scrapping these listings in favor of ones with URL's that match the business name. I can see that I can merge FB pages so am guessing that this would work for FB, has anyone any experience of this? Am assuming yelp will have to be deleted in some way. Any thoughts?0 -
Effects of changing your city to overall rankings?
One of our offices is in Trinity, Florida. However, "Trinity" isn't a real city. It is essentially the nicer part of New Port Richey. Organically, we rank for new port richey terms, but i have a suspicion, our lack of maps/local presence is hurting us. Should we do a massive citation overhaul and change everything to New Port Richey for that office? Will doing that somehow hurt our site overall? Thanks, Ruben

URL Structure for a local listing site
Hi - We have a site that offers customers with wide range of local businesses information. We have URL structure for those these days like this - site.com/listings/plumbers/ca/sfo However here the "listings" doesn't add any value and are confusing for users who want to remember it, hence we decided to make it cleaner. The question here is, which one of the two should we do and why? site.com/plumbers/ca/sfo or site.com/ca/sfo/plumbers Any help on this matter would help.
