I can't replicate this in live SERPs, but Matthew's explanation seems to match the evidence. It does appear that Google is doing some kind of implicit canonicalization. It's rare, but it does happen.
You could try giving the PDF a unique title in its own meta information (the document doesn't seem to have any). It's more likely to look like a duplicate if that's undefined.
The next step would be to recreate the PDF slightly and make it unique, but honestly, each step is going to get to be progressively more work. It really depends whether this is just an anomaly or it's causing you actual issues as a business.

I would say this is a duplicate content issue.
Identical versions of the PDF are found on both the sweetwater.com domain and also americanmusical.com:

Hmm.... This is wacky.
It looks like although it says "Sweetwater" in the Title Tag, the pdf file is still being hosted on your URL (as well as the actual rebate form having American Music listed as the return address).
Maybe perhaps whoever added this rebate form to your website "borrowed" it from Sweetwater? And by that I mean, copied over the pdf file, changed the address, and copied everything else (Title Tag included), but forgot to take off the Sweetwater brand name?
Either way, it's on your own URL and it's a rebate form that includes your own address. Just get into your site editor and change the title tag and you should be fine.

404's Wordpress products
Hi Guy's, On a Wordpress website we have a SEO Ultimate plugin running. Every day i get lot's of 404 errors of products that doesn't exist anymore (but are indexed, site: .... ). In the beginning we had lot's of testproduct that are not coming back in the shop. So i was wondering if there is a way to automaticly redirect product when there are out of stock, or not comming back anymore... So my 404's can be fixed. Thanks!

What's better for SEO a page per review or a page with all reviews?
Was wondering what's better for SEO. We have a platform where consumers can read and write reviews. But the question is: is it better to give one page per company with all the reviews on it? Or should we have different pages for the specific company? Example: Itunes has a company page with all reviews on the page, but not the whole review. You have to click further to view the whole review (new page), at the moment this the current situation. What if we place the whole reviews on the company page, so you don't have specific pages for the reviews? Hopefully can someone help us out. Rich Snippet Not Appearing? Why?
I run a price comparison website and am trying to get merchant name, product name and price appearing in SERPS I have used the Google Structured Data tool and after initally receiving some errors, I was able to remove the errors and ended up with what I thought would be my ticket for awesome looking SERPS. Are Title meta tag keywords case sensitive'
Hi! One of the fixups that SEOmoz analysis reports is that my main keyword isn't part of the page title. The problema is that the keyrord is there but in uper case while i introduced a lower case keyword for analysis (see the example). Strange meta description shown in SERPS
Hi there, I recently stumbled upon a strange phenomenon in the SERPs. Here's the situation: A customer’s website is ranking for a keyword with the root domain. Strangely the actual meta description (as implemented in the source code) is not shown. Instead Google takes the H1-Header and a part of a short element from a contact form. Any clue/hint, how this could have happened and can be changed? Sorry, that I can’t name the domain and the keyword (in German), but I hope, the above explains the problem. Canonical URL's - Fixed but still negatively impacted
I recently noticed that our canonical url's were not set up correctly. The incorrect setup predates me but it could have been in place for close to a year, maybe a bit more. Each of the url's had a "sortby" parameter on all of them. I had our platform provider make the fix and now everything is as it should be. I do see issues caused by this in Google Webmaster, for instance in the HTML suggestions it's telling me that pages have duplicate title tags when in fact this is the same page but with a variety of url parameters at the end of the url. To me this just highlights that there is a problem and we are being negatively impacted by the previous implementation. My question is has anyone been in this situation? Is there any way to flush this out or push Google to relook at this? Or is this a sit and be patient situation. I'm also slightly curious if Google will at some point look and see that the canonical urls were changed and then throw up a red flag even though they are finally the way they should be. Any feedback is appreciated. Thanks,
