What is the optimal image naming strategy for the following? Should it be A or B or something else?
a) disease name-topic name-treatment-studies by age
b) diseasename-topicname-treatment-studiesbyage
Thank you.

I wouldn't use spaces in my image names, just because they will get parsed out into "%20", and that will make the URL path longer and uglier.
Option B is the better one of the two. The most important thing to remember here is to just be descriptive. You're already on the right path.
Here are a few great articles on SEO for images. I think you might find them useful:

Optimizing Images
I have realized that our images are way to big. I would like to resize them and upload back again but I am afraid to do it as every image in wordpress is uploaded and has own link and then all this links will be not found. Is there some other way how to do it without a plugin. Or it is not a problem that I will delete that old large images and then upload the new smaller? Thank you very much for any advice Iris

Google Images
Hi, I'm setting up a Barcelona guide as a hobby: barcelonacitybreak.com. Naturally I take care to use good alt-tags, checking Google images I see one of the photos I took listed as number one: "hibernian bookstore barcelona". There is no doubt that this is the photo I took myself, but when I place the mouse over the photo the origin is listed as:http://www.auladirectiva.com/. What is even more worrying though is that when I press the image I automatically download this: "Antispy 2011" from http://protectionxpscannermaster.com! Evidently some kind of "anti-virus" program. How is this possible?
