Pushing for the Top 3 Spots

Getting a site listed on the first page for most keywords is fairly easy but pushing for the final 3 positions can be difficult.
Have you ever experienced a point where you feel you have given all you can for a campaign and are left scratching your head as to why your not taking over the competitors site ?
From experience SEO only tends to count once you reach those top positions.
When you reach burnout with a keyword with link building etc
Where do you turn ?

I look at "who" and "what" is above me.
Let's say that you rank #3 and two very informative pages on Google.com are above you then you need a great title tag to compete with them for the click. You might never beat them for this query so figure out a title tag that will pull away their visitors.
Let's say that you rank #3 and two decent articles on decent domains are above you... Beating their links will probably be difficult... but if you beat their content then you might have a chance.

Why my site doesn't come on top even though search terms are very similar or exact?

Root Domain Metrics Stagnant for 3 weeks now
We are doing a Competitive Domain Analysis for a client and 2 of their competitors. The results from the Root Domain Metrics have been ok from the past months where in the results we are getting changes every time the website is re-crawled by SEOMOZ to update its analysis. However, in the past few weeks, the results we are getting for the Root Domain Metrics have remained the same even after three weeks. We have attempted a separate Open Site Explorer analysis for the client and their competitors but be are getting the same results. From the "Domain Authority" to the "No Followed Linking Root Domains", the results have remained the same for three weeks. Could anything be affecting the results or is it just the fact that the links have not changed (not a single one, for three websites) in the past three weeks. Hope you can help me clarify my issue. Site in top 3 with a 1-day old domain and no backlinks

http://www.google.nl/search?sourceid=chrome&ie=UTF-8&q=online+casino&pws=0 You'll see as position 3 result this website: http://bit.ly/qhRsUT, it's obviously that this site is there because of blackhat. But how did it come there such high for a keyword with high competition? I don't see anything wrong in the website? It's not like Google did something wrong, because it happened already a few times for this keyword with different domains from that owner. Regards, Yannick0 -
Image only site on top of Google
Hi Everyone, I'm trying to rank in Google for 'Hid xenon' in the netherlands, but there is one site above all results: http://bit.ly/qlsjne As you can see the site almost has no backlinks, and has not a single word in it's content, all images. it's a keyword only domain, and that's probably the only reason why it's ranking that high, but that means then that i can never get higher then him in Google because of it's domainname? Google Places - Top Listing & Strange Analytics
Hello, we have been working with this customer for a few years, doing their PPC, organic marketing, and we had established one google places listing for them as well. I guess the owner got sold on having someone else work with us to do google places for an additional office location they recently set up, and for whatever reason, they bypassed having us do it. This company never gained FTP access to the website. And despite heavy competition (apparantly), they have that new location listed in the #1 - A spot, without making any changes to the website. And, to top it off, when you review the Google places performance, there is a weird result I had never before seen labeled as "* loc:". You can see what I'm talking in both screen shots. Is there any guidance you can offer, first as to what that listing label means, and second, do you have any ideas how to 'reverse engineer' how they were able to get top listing so quickly for our customer like that? local_results.jpg local_analytics.jpg
