How to Link sub-pages of a category without cannibalization?

What's the best practice to link to sub-categories without cannibalizing SEO?
Main page title = Dog
Sub page title = dog collar
Sub sub page = spiked dog collar
I used SEOMoz program and it showed the link from the main page to the sub page as cannibalized and also as duplicate title Same thing for the sub-sub page. How to avoid it?
Please provide examples
Questions:
- Would you use: dog_collar or dog-collar for the sub-page?
- How would you link to the sub page from the main page?
- How to title the sub pages to avoid duplicate titles?
- How would you link to the website from other websites? What kind of tags would you want used in the links?

Canabalising is a tricky thing.
My thoughts - and these may not be 100% correct! - are as follows;
1. dog-collar
2. All depends on how the navigation works for your site, Do you have lots of categories under Dogs? Just list the sub categories and call this one 'dog collars'. You should be doing stuff to make sense to your site visitors as well as worrying about Google, and this makes sense
3. Your titles can hold up to 70 characters - you have plenty of space to make sure the titles are not duplicates even if they both start with Dog. Just make sure you use all your space and have it read well and not 'stuffed'
4. I would use anchor text that makes sense for the readers of wherever the link is being served. There has been all sorts of discussion on too much exact text anchor linking being penalised, but if you keep the readers in mind you should be OK. ie use the website URL when it makes sense. Or 'dogs', or even 'spiked dog collar' if you are actually describing it or talking about it on the website.

I see some links on Youtube.com that look like they are "Do-follow" links am I wrong?
Hi, I have some videos on YouTube.com and I notice many of the links show as "Do-Follow" per the Moz Bar. I have a feeling they are not actually "Dofollow" links because they never show up in Moz Pro, SemRush or any of my other tools. Keyword stuffing - on page grader count
I used the on page grader for my homepage, www.cprnj.com, and it said the keyword "physical therapy" was used 26 times in the body of the page. But I can't find more than 4. How exactly is keyword stuffing accounted for, and is there some place that I am not seeing the keywords?

Why would I be ranking for a term when it's not anywhere in my page or pages linking to it?
Hi, I have a keyword that's not anywhere in my page in page-grade tool nor are any pages linking to it with that term, why would that be ranking. The term is "dynamic card solution" This is a company we used to have 5 years ago and merged with our company. This term is ranking #1 for this page- http://www.datacard.com/instant-issuance-solutions . I can't figure why that would be even as this is a newer page in the past year. Tool recommendation for Page Depth?

Question on redirect and internal link count
After crawl I saw that dup content came from domain.com/index.php and domain.com. I was going to redirect one of them but im curious which way to go. The /index.php has domain auth of 19, the other 17, so not big difference. Domain.com has 2 external linking domains and index.php has 1. The big difference is in the internal link counts where index.php has 76 and the other only 4. So should I direct index.php to / or the other way around?

Page Ranking by URL / Keyword
Needing to know how to find out the page rank of a URL that is NOT within the top 50 or top 100. Need to know that specific page's rank, not what our overall site's ranking for the keyword is. Can't seem to find any tool that goes beyond the top 100. Any ideas?

Open Site Explorer Page Authority
Hello, I ran a report on my website at http://www.opensiteexplorer.org/links?site=www.carrentalbuddy.com.au and noticed my page authority went to 1. I also noticed a lot of our results that were showing high page authority have dropped right down.
